Request for sponsorship or donation
Delta Dental of South Dakota is a nonprofit company dedicated to improving oral health. Our sponsorships and donations are focused on the three priority areas and must primarily benefit the people of South Dakota:
- Oral & overall health improvement
- Education & learning
- Charity & community
Note that there are separate forms for toothbrush donation requests and oral health grant applications.
We are unable to support events/projects outside our focus areas. Please do not submit sponsorship requests for:
- Political organizations or campaigns
- Projects that exclusively serve religious purposes
- Organizations that discriminate by race, religion, color, creed, gender, or age
- Individuals
To request a sponsorship or other donation, please complete and submit the form below. We will review your application and respond within 30 days.
Please submit your request at least one month before the date you would like to receive it.
