Change of information 

It is always our goal to process claims from your office quickly and accurately. We also want our members to easily find your office in our Find a Dentist directory. Help us by promptly reporting changes occurring in your office.
 
Please let us know if:
  • Office contact information has changed (email, phone number)
  • The office has moved or has an address change
  • The office has a new bank account or EFT contact email
  • There has been a TIN change
  • There is a dentist change – someone joined the practice or left the practice
  • There are other changes that may impact claims payment or directory listing
