Please let us know if:
- Office contact information has changed (email, phone number)
- The office has moved or has an address change
- The office has a new bank account or EFT contact email
- There has been a TIN change
- There is a dentist change – someone joined the practice or left the practice
- There are other changes that may impact claims payment or directory listing
